Assisted reproduction in 32.1% of pregnancies, far more often in PCD (48.5% vs 8.7%). Lung function stayed stable during and after pregnancy; exacerbations occurred in 58.6%.

Post hoc analysis of a 2-year prospective study in adults starting mepolizumab, split by allergic / omalizumab-eligible phenotype and type 2 biomarkers (eosinophils, IgE, FeNO). OCS use fell across phenotypes.
- [Nerandomilast-associated pneumonitis with a positive rechallenge: a case report](https://doi.org/10.1002/rcr2.70786): Respirol Case Rep, 2026-10-19. First reported pneumonitis on nerandomilast. A 33-year-old with unclassifiable IIP developed fever, hypoxaemia and new bilateral consolidation and ground-glass opacities 9 days after starting the drug; it recurred the day after a reduced-dose rechallenge and resolved again on withdrawal. Worth remembering as the drug enters practice.
- [Indirect echocardiographic signs of increased right-sided afterload in progressive pulmonary fibrosis](https://doi.org/10.1007/s00408-026-00955-2): Lung, 2026-10-18. 158 patients with PPF across Italian centres. 19% had at least one indirect echo sign of RV afterload, most often a short RVOT acceleration time. Only male sex stayed independent on multivariable analysis.

## Defining 2026: papers changing practice this year
- [Tozorakimab (anti–IL-33) in COPD: the OBERON and TITANIA phase 3 trials](https://www.nejm.org/doi/abs/10.1056/NEJMoa2606998) (N Engl J Med, 2026): Fewer moderate-to-severe exacerbations with IL-33 blockade, including in patients with low blood eosinophils, which opens a biologic option beyond the type 2 phenotype. Not yet approved.
- [Nerandomilast in patients with idiopathic pulmonary fibrosis (FIBRONEER-IPF)](https://pubmed.ncbi.nlm.nih.gov/40387033/) (N Engl J Med, 2025): 1,177 patients, 78% on background nintedanib or pirfenidone. FVC change at 52 weeks −114.7 mL with 18 mg bid vs −183.5 mL with placebo (difference 68.8 mL, P<0.001). Diarrhoea 41% vs 16%. First new IPF drug in over a decade.
- [Inhaled treprostinil for idiopathic pulmonary fibrosis (TETON-2)](https://pubmed.ncbi.nlm.nih.gov/41812190/) (N Engl J Med, 2026): 593 patients, 75% on background antifibrotics. FVC difference 95.6 mL at 52 weeks (P<0.001); clinical worsening 27.2% vs 39.0% (HR 0.71). No effect on time to acute exacerbation. Cough 48% vs 24%, discontinuation 34% vs 25%.
- [Budesonide–glycopyrronium–formoterol in uncontrolled asthma (KALOS and LOGOS)](https://pubmed.ncbi.nlm.nih.gov/41692019/) (Lancet Respir Med, 2026): 4,311 patients uncontrolled on medium- or high-dose ICS-LABA. Trough FEV1 +76 mL vs budesonide-formoterol; severe exacerbations IRR 0.86 (vs Aerosphere BFF) and 0.82 (vs Symbicort). Benefit held regardless of recent exacerbation history.
- [Phase 3 trial of the DPP-1 inhibitor brensocatib in bronchiectasis (ASPEN)](https://pubmed.ncbi.nlm.nih.gov/40267423/) (N Engl J Med, 2025): 1,721 patients. Annualised exacerbation rate 1.02 (10 mg) and 1.04 (25 mg) vs 1.29 with placebo (rate ratios 0.79 and 0.81). First therapy approved specifically for bronchiectasis.
- [Sotatercept in pulmonary arterial hypertension at high risk for death (ZENITH)](https://pubmed.ncbi.nlm.nih.gov/40167274/) (N Engl J Med, 2025): 172 patients in WHO FC III–IV, REVEAL Lite 2 ≥9, on maximal background therapy. Death, transplant or PAH hospitalisation 17.4% vs 54.7% (HR 0.24). Stopped early for efficacy.
- [Mycophenolate and azathioprine in fibrotic interstitial lung disease](https://academic.oup.com/ajrccm/advance-article-abstract/doi/10.1093/ajrccm/aamag362/8732287) (Am J Respir Crit Care Med, 2026): Target-trial emulation in 2,270 patients. No survival or FVC-trajectory benefit in any subtype; worse 3-year transplant-free survival in non-IPF IIP (HR 1.38) and fibrotic HP (HR 1.62). Observational; RCTs needed.
- [Biologic management in severe asthma for adults: CHEST clinical practice guideline](https://pubmed.ncbi.nlm.nih.gov/41005695/) (CHEST, 2026): Seven PICO-based recommendations. Choice guided by QoL, lung function, exacerbation frequency, maintenance OCS, endotype, biomarkers and comorbidities; no head-to-head data. Consider switching after a poor response at 4–6 months.
- [Chronic thromboembolism phenotypes after acute pulmonary embolism: the SEARCH algorithm](https://pubmed.ncbi.nlm.nih.gov/?term=10.1183%2F13993003.01562-2025) (Eur Respir J, 2026): 150 patients ≥3 months after PE, stepwise CPET, perfusion scan, echo and rest/exercise RHC. Recovery 66%; chronic thromboembolism phenotypes 12%, mostly without PH; CTEPH 2.1%, CTED with exercise PH 3.5%.
- [Inhaled marijuana and asthma attacks: nationally representative survey data (PATH)](https://pubmed.ncbi.nlm.nih.gov/41850484/) (CHEST, 2026): 4,477 adults with asthma. Smoking cannabis was associated with OCS-treated attacks at one year (OR 1.64, 95% CI 1.12–2.40), with signals for vaping too. Observational.
- [Three versus six weeks of corticosteroids for mild immune-related pneumonitis: a randomised trial](https://pubmed.ncbi.nlm.nih.gov/42398004/) (Am J Respir Crit Care Med, 2026): 106 patients with grade 1–2 ICI pneumonitis. Success at 8 weeks 66.7% vs 85.2%; non-inferiority of 3 weeks not shown, exploratory superiority of 6 weeks. Keep the guideline 6-week taper.
- [High-dose pulse vs standard-dose methylprednisolone for acute exacerbation of IPF: a randomised trial](https://academic.oup.com/ajrccm/advance-article-abstract/doi/10.1093/ajrccm/aamag529/8853809) (Am J Respir Crit Care Med, 2026): 100 patients in South Korea. Three-day pulse did not beat a 7-day standard dose: mortality 10% vs 15% at 28 days and 25% vs 30% at 90 days, not significant.
- [Acute exacerbation in fibrotic interstitial lung disease: an international working group report](https://pubmed.ncbi.nlm.nih.gov/42447235/) (Am J Respir Crit Care Med, 2026): Extends the 2016 AE-IPF definition to all fibrotic ILD: acute event with DAD-pattern imaging or histology. Adds an acute respiratory worsening framework to separate AE from infection, oedema and other causes.
- [Nonadherence to inhaled corticosteroids is associated with negative mepolizumab response](https://pubmed.ncbi.nlm.nih.gov/42898776/) (ERJ Open Res, 2026): 190 adults with severe eosinophilic asthma. ICS nonadherence in 52% of non-responders vs 22% of responders, with higher FeNO when nonadherent. Read with SHAMAL: on benralizumab, 92% could step ICS down, though not to zero.
